Instead of taking time to contact the National Office to enquire about relevant Application Forms, these forms are now directly available for download from this webpage. We trust that it will make our church administration more effective to have these forms readily available. Scroll down to search for the Application Form you are looking for and download it in PDF format. Forms are electronically editable and can be completed within the PDF document (without the need for printing it out). Completed forms can then be saved and e-mailed back to the AFM National Head Office.
